New Windshield Card Form - Waterfowl

Use this form to print a new Waterfowl Windshield Card for a new site.

To replace a lost or damaged windshield card, please use this form.

Not all species are legal to hunt or trap at each site.

Prior to printing your windshield card, consult the Hunter Fact Sheet for the site you are choosing to determine which species and time frame you can hunt or trap.

Numerals only - enter as on hunting license

3 groups of 3 digits, include leading zeros if applicable.

Please select the Site in Northeastern Illinois where you will be hunting.

Each site will require a separate windshield card.

Note: Hunting areas with limited numbers of hunters or special hunt drawings will require a windshield card to be issued form the site office where you intend to hunt.
